Services provided: - Light housekeeping assistance, including laundry, changing bed linens, tidying, and cleaning up of regularly used rooms in the home • Personal care assistance to help with bathing, dressing, toileting, transferring, continence care, and feeding • Assistance with mobility – walking in and around your home • Medication reminders • In-home meal preparation assistance • Assistance with appointments errands – side-by-side assistance throughout your appointment schedule or local errand route • Extra planning and care for those needing memory support Our goal is to provide you with quality and consistency of care. We accomplish this by following a specific set of care planning steps prior to the start and throughout your service: 1. Conduct a comprehensive needs assessment 2. Develop a care plan outlining specific tasks, frequency, and details 3. Select your care team based on the skills needed to provide the service 4. Provide training and shadowing by our lead caregiver when needed 5. Outfit each caregiver with a mobile app to access specific care plan details 6. Ensure caregivers clock in and out using the mobile app 7. Log and track tasks completed 8. Check in within the first few days to see how it is going 9. Report changes to the nurse supervisor and revise the plan Blakeford At Home care is provided by a team of experienced caregivers who are carefully selected and trained to meet the professional standards of Blakeford Senior Life. As Blakeford employees, not independent contractors, all team members are covered by Blakeford’s liability insurance and are supervised by a licensed, experienced nurse. EACH CAREGIVER MUST: • Pass rigorous pre-employment screenings • Meet all state and federal qualifications for employment by Blakeford • Demonstrate competencies in a core set of skills • Participate in an extensive orientation • Engage in regular hands-on in-services • Demonstrate a passion for working with older adults
