Key Responsibilities: Provide Quality Care: Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility, ensuring a safe and comfortable environment in their homes. Follow Care Plans: Implement and follow personalized care plans tailored to each patient’s unique needs and preferences, while adhering to all homecare standards and regulations. Monitor Patient Health: Track and report changes in patient’s condition, including vital signs, physical, mental, and emotional status. Communicate effectively with the care team and family members to ensure optimal care. Provide Companionship: Offer emotional support and companionship to patients, fostering a positive and nurturing environment. Maintain a Safe Environment: Ensure the patient’s home is free from hazards and follow all protocols for infection control, safety, and cleanliness. Document Care: Keep accurate and up-to-date records of patient care, including daily activities, changes in condition, and any incidents, in compliance with company and regulatory standards. Qualifications: We have a variety of CNA and HHA openings available and are excited about matching you with a great opportunity!
