Home Health Aide - Urbana, OH

Qualifications: • Must be 18 years of age or older • Minimum High School graduate • 1 year of work experience in any field • Ability to lift up to fifty pounds • Valid state driver’s license • Reliable means of transportation • Current CPR certification • First Aid certification Responsibilities: • Follows the Home Health Aide care plan and/or assignment form which has been completed by the RN • Assist the client with the activities of daily living. • Provide and maintain a safe environment for the client. • Plan and prepare meals. • Perform routine household tasks as related to a safe and comfortable environment. • Provide optimum physical and emotional environment. • Promotes independence according to the nursing care plan • Document the client’s condition and care daily in the clinical record. • Submit clinical notes weekly, and other clinical record forms outlining the services rendered as indicated. • Report to the Clinical Supervisor any change in the client’s mental or physical condition.
