Responsibilities Conduct initial patient assessments and ongoing evaluations to monitor health status Assist in developing individualized care plans in collaboration with healthcare teams Provide direct patient care, including administering medications and treatments as prescribed Educate patients and families on health conditions and care plans, fostering understanding and compliance Document patient progress and maintain accurate medical records in compliance with regulations Communicate effectively with physicians, other healthcare professionals, and family members Technical Skills and Relevant Technologies Proficiency in using medical equipment and technology relevant to patient care Basic computer skills, including familiarity with EHR systems Soft Skills and Cultural Fit Empathetic and patient-centered approach to care Strong problem-solving skills and the ability to adapt to changing situations Team-oriented mindset with a commitment to collaboration and communication
