BETHLEHEM WOODS NURSING AND REHABILITATION

Our mother has lived at Bethlehem Woods Nursing Home for the past three years. The caregivers and other staff treat her with kindness, compassion and respect. Each and every day her needs are taken care of and she enjoys living there. The facility provides many activities to keep her engaged throughout the day including games and musical acts. She loves the food, which is very important to her. They even have a hair salon on site to help keep her looking and feeling good. This year they hosted a holiday dinner at the facility so family members and residents could enjoy a holiday meal together. We have become friends with many of the staff members and are grateful for their professional manner and the loving care that they provide. We feel blessed to have found such a great facility for her to live in and appreciate everything that they do for her and the other residents.

I just visited this nursing rehab facility for the first time recently. They are tucked away in a beautiful wood setting. I even saw some deer close to the windows. They are VA connected and have a really nice VA honor wall. The memory care area was very peaceful and the director was making fresh bread and it smelled so good. I would highly recommend them for your loved ones needs.

My father needed a place for rehab. This was the second facility within a few months. He was VERY happy there with the staff and the FOOD!! Believe it or not, food is so important. Especially when they lose their appetite. Living in Florida makes it difficult to make sure he is receiving proper care. I talked with him every day and he never had one complaint about the staff or the food. He received excellent care and physical therapy. I had an upright walker delivered there and the physical therapist worked with him using the walker before his release. Our plan for the future is for him to return to Bethlehem Woods when that time comes. Thank you sooo much for taking such excellent care of my father!! It's EXTREMELY important to me as I continue to keep honoring his wishes to stay home as long as he can with home health care. We will be coming for a visit when I return in July. Thank you again!!! Update 10/8/2025 My father is currently undergoing his third stay at Bethlehem Woods. I would like to express my sincere appreciation for the consistently exceptional care and support provided by the entire team. Specifically, I am deeply grateful for Ms. Val's kindness and welcoming demeanor, the attentive staff at the nurses' station, the physical therapy team and the effective leadership of the management team. Their dedication has significantly contributed to my father's well-being, and I am truly thankful for their commitment to his care.

Only Nursing and Rehabilitation center that accepts VA Benefits in Fort Wayne! Bethlehem Woods Nursing and Rehabilitation is a beautiful, quaint facility with a woodland environment right off of St. Joe Center Rd. Only 8 minutes away from Parkview and DuPont hospitals. This home-like environment changes the mindset on a typical “nursing home” with music playing throughout the halls and a beautiful wilderness mural that enters into the memory care unit.

My mother and aunt were both in this 3 ring circus! Both with dementia and told Chris they couldn’t be together because it was draining my mother so he moved her to the regular nursing home part where she was doing very well and Chris moved her room every 2 weeks which confused her lots and he told us the last time he moved my mother back into memory care that if this didn’t work out we would have to find a different facility. not once did they have a health care plan meeting with me or my sister for either one of them!!!! My aunt fell either out of bed or the couch they were letting her sleep on and nobody witnessed it and didn’t notify my sister until 17 hours after the fact which she had fractured her hip and had to have surgery. Lost some of their clothes. Nurses yelling at the memory care patients to get back in their wheelchairs, no patience with them at all!! My aunt broke her glasses and we weren’t notified about that either. When Chris found out we were moving both of them out of that facility he said his staff had been retrained! Well I’m sorry but if you have to retrain your staff you didn’t do a very good job of training them right the first time! He didn’t even realize my mom and aunt were sisters until we were moving them out and the rest of the staff knew! Like I said 3 ring circus!!! Please NEVER put your loved ones in Bethlehem Woods! Sense moving my mother and aunt to two different places they are both thriving!
