I have never seen a place quite like this. My mother-in-law, who has end stage breast cancer is a Providence Medicare advantage patent. After being placed there by Providence Saint Vincent’s case manager, who stated she had a good relationship with Avamere but had never been to this particular facility. Possibly she was thinking on the Pearl on Kruse Way which is another Avamere facility. My husband, in good faith, dropped my poor mother-in-law off there. She was there for 5 day rehabilitation session because of weakness in lower extremities. She was placed in a two person room a with long term patient divided by a curtain that was stained with looked to be old blood. There was no bathroom open to the patients in the room although my spouse could access it and used it from the other side. He thought maybe because the nurse manager’s office was on that side and maybe it was for office use only. She was given a bedside commode along with her long term roommate. She had no access to a shower either during her stay nor was she offered even though at Providence was allowed to use the shower. Clackamas Avamere placed her on a mechanical diet when she had a regular diet at Providence. The food was inedible we took pictures of it and we brought her in food in from various restaurants. The place smelled like feces and necrosis. I do not believe they ever have time to deep clean the rooms. I have never been at a place that dirty in my entire life. Finally, saw someone cleaning the hall carpet on Tuesday of this week who kept going over it with a portable rug cleaner. In addition, we had to beg for a physical therapy evaluation which was the reason she was sent there in the first place. I can not make this up and they even had the audacity to say she might not get q day physical therapy because they were on vacation!!! Fortunately, we were waiting for a one bedroom to be completely remodeled at a private pay facility. I have never had to experience anything quite like this in my family because of private pay and the ones that you buy into (CCRCs) for independent, assisted and skilled nursing. I truly feel sorry for families who can not afford private pay and have to go on Medicaid and have to actually stay at this skilled nursing facility which should be torn down. Where is CMS when you need them???? The reviews are definitely telling the truth! One more thing Avamere is for profit vs a 503c.

The staff here are unable to recognize when a patient has an infection. So if you have a fragile relative staying here, know that you will need to supervise them carefully yourself. Luckily I drove my relative to the ER myself—where he was immediately admitted to the ICU—all while the staff still seemed to think he was fine. Also, as I was clearing his belongings out of his room, the room was flooding from a burst pipe. The building is from the 1950s and falling apart. Some of the CNAs and therapists are lovely kind people. But staff do not respond quickly to the call button. As far as I could tell after two weeks of having a relative here, the is no supervising doctor involved in patient care and they don’t have any nurses on staff. If you try to ask any nurse a question, they will tell you that they don’t work there and don’t know anything about anything. Nobody will ever call you or proactively give you any information. It’s a very passive place that seems to be run by temp agency staff.

Staff are respectful and treat residents like family. They have a lot of patience and compassion. They go above and beyond to make residents comfortable and happy. They are hard workers. Bless them all. My mom was a feisty Chicago woman. She was not the easiest person to deal with. But they “got her”. They understood there was a kind soul under that tough exterior. That meant the world to her family that they saw what we saw in our beautiful mom.

We had to have our mom at a rehab facility after surgery back in April for a few months. Since the rehabs closer to us where full, this Avamere was recommended and we couldn't be happier with the care that was provided to her during her stay. Crystal Gage ,the director of social services, was an amazing person who kept us informed about everything our mother needed and was going through during the difficult time with pandemic. She made it easier for our family with all the help she provided. In addition, Andrea one of the physical therapists, worked tirelessly to help our mom regain some of her mobility back. Even though we never got to go inside because of covid, we could tell that everyone who works at the facility really cares for the patients and they go above and beyond to help them and their families.

I have had the necessity to stay in a Skilled Nursing Facility. In the last 2 years, I have stayed in 4 different places. My experience at Avemere Clackamas has been wonderful. The Staff (Admin, RN, CNA, CMA, Activity Dir, Dietitian, Kitchen Manager, Therapist ) all have been very nice and attentive to my needs. One of the Med Techs (CMA) Crystal, does everything in her power to show kindness, respect, and empathy. The kitchen Manager (Betty) has come to me periodically to make sure that my food needs are being met completely (and I have quite the list of needs). The other difference between this location and the other facilities is the quality of the food. Other places were bland and cold. This location has the best food overall. Chris is one of the Chefs… and he really knows what he’s doing. I haven’t loved everything, but that’s just a preference thing, and I have enjoyed prox 95%. The chefs here know how to season your food. The Activity Dir (Chelsea) has fun things planned daily, and she doesn’t just leave it up to you to check out what to do, Chelsea goes around and reminds all residents and helps them get to the adventures location. I could go on singing their praises, but suffices to say … I highly recommend Avamere Clackamas. Thank you for reading this whole review.
