Let me start by saying if you take new clothes to laundry to be tagged for your loved one make sure you take pictures , you will not see them again,next room theft don't even leave a pack of cigarettes out in plain sight they will be gone . Next some not all staff is just rude no hello no smile, they gossip about inappropriate stuff no one else needs to hear . Needless to say we will be moving them out asap. Front desk is amazing 👏 very nice

Do not put anyone in this disgusting place. The rooms are filthy. Food is disgusting. The patient care is even worse. Weeks pass between showers. Patients sit in their own waste as those don't get changed regularly and then sores develop. The sores need ointment but good luck getting anyone to do that. Please go elsewhere. You will regret putting your loved one in this facility for the rest of your life.

Very disappointed in management. There is no communication. They move people around so much the staff cant even find their residents The receptionist didnt even know who the director was when I called. My cousin who could not speak for herself received no PT even though that's why she was released from hospital. Didn't receive meds regular so had sseizures. Had to ask for clean sheets, water, and diaper changes when I visited. Made me wonder what kind of care was received when I couldn't visit. Floors were disgustingly dirty most of the time. So uninviting. Dont put anyone you love there.

Not once but twice my father was sent here and it was the worst decision we have ever made. Our father has been very sick this year he was sent here For rehabilitation sadly that never happened he laid in a bed for weeks whenever we tried to get a hold of him it was impossible. Our Father eventually suffered a very bad blood infection due to his  catheter not being cleaned or checked his stomach began to fill up with blood and pus. He was sent to the hospital to be treated to only get sent back to this place. We're not only he wasn't being taken care of but to fall out of the bed because there are no railings only Matt's on the floor for "his safety"... We went to see him and our father wasn't well we knew it... We asked for him to be sent to the emergency room for them to deny us our request they said there was no reason later that day he was sent to the emergency room after we asked 5 times that day for him to be  sent. Not because we asked but because they did a PICC line and got the test results back that his vitals were not good. Overall I can go on but I won't it's just nearly impossible to get your loved ones things back and they won't be taken care of don't choose this place please.

My mom was at this facility from April 2018 until June 2020. Everything was great from April 2018 until February 2020 when Carriage Rehab bought the place. My mom fell April 11 2020 due to lack of staffing. If there had been more staffing that night she would not had fallen. From that day on, her falling and Covid going on she gave up. After new administration came in a lot of good help quit and are still quitting! Agency staffing brought in because that’s all the help they can get. I recommend that if you’re looking for somewhere to place your loved one look somewhere other than here.
