I came for rehab from March 3rd to April 30th. All the people were very kind. Becky in Entertainment really gets involved with the patients, and makes you feel welcomed. Austin helps solve any problems you need fixed and Melissa is an awesome Case manager. The ladies overnight are like angels. Overall, my rehab stay was very beneficial to my health. Thank you!

Please don’t bring your loved ones here. Visited my precious grandma and was disgusted by how awful the staff are here. They are the rudest ‘health care’ workers I’ve ever encountered, although I’m not sure they care about anyone’s health at all. My grandma is not well taken care of. They brought us a poopy shower chair with scum and skin flakes on it. They have the wrong patient’s chart in her room- extremely dangerous. I voiced my concern to the DON about my grandma being dehydrated- I was ignored, talked down to, and told to focus on the bigger picture. That evening my grandma ended up in the hospital with sepsis and dehydration. When you approach the staff with a question or in need of assistance they respond. “What do you want”. They forget to feed her too and leave the food she requested on the nurses station- nurse didn’t bring it. We had to go looking for her food hours later. I tried to give these people a second chance. But each day they just proved to be more rude, and careless. Zero starts. Owner- don’t waste your time responding. Fix your staff.

ZERO stars. This place looks dumpier than a jail. I came to visit my grandma who is here for rehab and there was another patients chart information taped in the room. This could cause major medical error!!!! The DON is absolutely terrible. We had some concern about my grandmas condition and the DON was very rude, blamed us, and refused to offer help. I find this place and the DON to be extremely unprofessional and disgusting.

Debated on leaving a review, however, I think it's extremely important that anyone who ends up having to send their loved ones here know that they need to be their biggest advocate and be on top of everything. Horrible experience for me in regards to my father's time here, until he passed early July, (which the story given in regards to that event doesn't add up either). Soo many negative things to write about, but don't have the time to get into them all. There were a few wonderful staff members working there, and I appreciate them for their kindness, empathy & understanding to/with my father during his last months of life... However, unfortunately, our overall experience with this place was negative and it saddens me this is where my father spent the last moments of his life.

The worst place to have your mom or dad in!! It was much better decades ago when my Grandma Martha was there, then it was called River Pines. I wouldn't put a dog in thar place with what I know now!! Heartless cna's when my Mom passed in January of 2015, after she fell forward onto the floor of her room, facedown onto the carpet, our dear Lord only knows how long she did lie there helpless to get back into her wheelchair!! She was 90 when she passed, and she knew how to push her buzzer for help, believe you me!!!! I got told 2 different versions of how it happened, first one, Oh, she was trying to get from her chair to her bed!!", second one, "OH, she was trying to get from her bed to her chair!" MY foot! She couldn't walk due to the mini stroke she suffered in January of 2008!! And their insurance person, at the time, Mr. Mike Carmody, was sweet as molasses at first, OH, don't worry MS B, we don't want to keep insuring this nursing home, if they're negligent!!" Bull!! Liar!! I got a lawyer from Madison to sue them, not for the money, but so no one else had to have their mom or dad go through what my Mom had to!! From the fall, she had a broken pelvis and femur bone in her leg. As I said, I would not put a dog in thar horrid place, better yet, a rodent shouldn't live there!!! 😡
