Description
CareGivers America, a Modivcare Personal Care Service, is looking for an RN/LPN responsible for providing high-quality services to our home care private duty clients. We are seeking a compassionate, attentive, and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one care for a bright and resilient 12-year-old girl in her home. She is recovering from a significant neurological injury and requires ongoing medical support, monitoring, and assistance with daily activities. This role is ideal for a nurse who is patient, observant, and confident in pediatric care, and who values building meaningful connections with their patient and family.

Primary Responsibilities
- Provide skilled nursing care in accordance with physician orders and care plan
- Perform ongoing neurological and physical assessments
- Monitor vital signs and observe for subtle changes in condition
- Administer medications and treatments as prescribed
- Assist with mobility, positioning, and safety supervision
- Support feeding needs and ensure adequate hydration/nutrition as directed
- Provide assistance with activities of daily living (ADLs) as needed
- Maintain accurate documentation of care provided and patient response
- Communicate effectively with parents, physicians, therapists, and care team members
- Promote a calm, supportive, and structured healing environment
Ideal Candidate
- Current LPN or RN license in good standing
- Pediatric experience preferred (home health experience a plus)
- Experience with neurological recovery or complex medical needs is beneficial
- Strong assessment and critical thinking skills
- Gentle, patient, and emotionally supportive demeanor
- Ability to remain calm and responsive in urgent situations
- Reliable, professional, and respectful of family privacy
