Most of the residents are honestly awesome and a pleasure to take care of, however nursing staff has ZERO support from management and are over work to the max without even a thank you. Most of the nursing team is chill and supportive of each other, but there is a large group of nursing staff who pretty much gossips and lies on the coworkers they don't like which makes it an extremely toxic work environment and makes you want to quit even before you're done with orientation. Pay is not great and benefits are horrible just like in any other nursing home nowadays. ALWAYS short staff specially with nurses. The DON Charlie is rude, lazy, mean spirited and honestly USELESS when it comes to supporting her staff. Beware of LVN Zee, she is the one of the worst human beings you will ever meet in your life, she demands respect but is disrespectful to most of the staff, she feels like she in a position of power and control but she's just a NOBODY just like the rest of us lol. Medpass is not crazy heavy but facility doesn't have working laptops for you to use and pass meds. Overall I wouldn't recommend this place to nobody unless you like being stress beyond believe with little to no reward. KEEP LOOKING

The staff at Meadowwood are amazing,kind understand and are always taking care and talking to there patients like they are family. From the short time I was there visiting my brother who is terminally ill, and on his way out of this life ready to go meet the Lord he is very happy there. The staff treat him like family. They get him to smile and laugh and generally all the patients seem to be happy when they're in their wheelchairs watching TV or just moving around, the people are so polite from the staff to the patients. I believe Meadow Woods to be a good place to send anyone in your family who's needing nursing care I give them a five star review I feel as as a believer in Jesus Christ, that the light of God is shining on this place and taking care of his children as one would want their loved one to be taken care of

I can’t stand this place. It breaks my heart every time I have to leave visiting my mom here. Today was a fast visit as I have so much things in my own life 40 hour a week job and I have my child who is suffering from migraines and has epilepsy. We think we can trust these homes to take care of our loved ones. I love a handful of nurses who have been amazing but they switch it up so much. First they didn’t keep up with her bowel movements and her intestines got tied up into a knot she got sepsis had to go to ER and was hospitalized for weeks. Almost died and even the nurses told us it was their faults for not documenting that she had no bowel movements. She can not walk now so she has to get help by a hoyer which she is terrified of getting dropped. Now she has bedsores from which they explained to us is from sitting I. Her pee and poop and not getting rotated or out of bed. We ask every weekend for her to get up and ready in wheel chair so we can take her for a walk around the block and out back to sit, last weekend we couldn’t even make it halfway because they don’t give us chair with no leg lifts so she kept sliding almost out of chair was horrible. Today we tried to call all morning to ask to get a chair ready but no one would answer. So no chair we just got to bring her food, because I always bring her food. Her food they feed her looks like cat food frozen veggies and a mystery side🤷🏽‍♀️ liquid protein which she hates. So I always ask her what she wants and bring it. She wanted shorts on and I noticed her feet nails were yellow and so long and I decided to check her hands and she has fungus eating through her nails. Attached are photos. I’m sure your asking your self why we have not moved her, but we have tried. Numerous times and the way they draw up her care is a paraplegic and no one thinks they can care for her. So she’s stuck here at this place that’s just letting her decay. I wish I could bring her home. I wish I didn’t live in an apartment upstairs that is a town house. I pray everyday that we get her closer to Ukiah where I live and can go everyday to help take care of her. Meadow wood is not a good place. The only time they reached out is cause they let her insurance lapse and they want to get paid. So disgusted by this place I have no words and the poor nurses who actually give a crap are over worked and treated terribly that’s why the turnover rate is so bad. Cause if they say something next thing we know they are gone. Terrible place I am gonna take pictures for now on to document everything.

Meadowood is a very clean facility and the staff are very compassionate towards their patients. My only complaint is they are very difficult to get on the phone when I call. Other than that, we are very happy with their services.

This place is disgustingly gross. My mother was placed here after a hospital stay. She was supposed to have physical therapy and rehabilitation but instead my mom ended up worst being here than she was when she left the hospital. I come to visit my mom weekly and almost EVER TIME I come to visit she smells of body oder, has greasy hair/skin, smells like pee from sitting in her own, dehydrated, and she is emotionally withdrawn besides wanting to get away and out of there. There were multiple times where I have asked for her to be showered but the nurses insist she has certain shower day so they are unable to shower until then. Which ok but obviously from her stench, greasy hair and face she hasn’t been showered on her scheduled dates. They refused the first time so we complain. On the second occasion I asked, the shower team came to me and said they would but after lunch time but would be after I left so I said ok. When I got home, I called my mom around 3:30pm that same day and per her and her roommates no one had come in to shower her, typical. She ended up getting bed sores which lead to her having a skin graft which they did not notify family about until after the fact. But yet knowing she has a skin graft, I would still come in for a visit with her sitting in her pee. There were a few occasions where she didn’t even have a diaper or panties on and her gown was soaked in pee. Which I reported and left a review through their sign in system, through the link that is sent to you every time you signed out. Well now after my last horrible review, I no longer receive those links to leave a review ( no need to wonder why, very clear as to why). Which Jimmy tried to reach to me to discuss but I knew it was them calling and this is the dude that helps prevent my mom from transferring. My family has been more than open about how we feel about this place. We have been trying to get her out of there and transferred elsewhere. But come to find out from talking with other facilities (which we know and talked with people who work there) Jimmy who sends the referrals was never actually sending any out, none were received by any facility we requested. Which we told admin so eventually he did send a request but in the transfer request he claims my mom is paraplegic and claims that some facilities don’t have the ability to care for her. But he failed to mentioned that they allowed her to get like that. She got no physical therapy which they claimed was due to my mom not liking their therapist which fine ok. So we requested the assistant to help her out which they agreed to. But when asked if and when physical therapy takes place, they have no records of physical therapy ever happening with the therapist assistant. My mom’s multiple roommates have all confirmed that my mom has yet to have physical therapy. On top of this, they claim that her insurance lapsed so there was a pending bill for her stay, which were like ok we’ll get her insurance situated and they’ll go back 3 months and will pay it. But in the mean time they have been calling my sister asking for my moms tribal money for payment which once again we said no, her insurance will cover it once situated. They proceed to ask my sister to make sure her contact information is updated so she did. Within weeks of updating my sisters contact information, she was sent the bill for the relapsed time of insurance. This place kills people, uses them for money, and prevents any future for anyone who needs help mentally, physically, emotionally, and financially. Not to mention how disgustingly dirty it is. This place needs to be shut down which I am in awe that it hasn’t already. Everyone here leaving a start or more rating is completely lying or has a family member that works here so they’re helping with reviews. No way in anyone’s right mind would this place even have a 1 star rating. I’m sure I’ll be hearing from them as soon as I post this, leave it all in the voicemail for records Jimmy.
