My brother-in-law, with a traumatic brain injury and heart condition, was placed here for skilled care. • Day 1: He jiggled the front doors and walked out. Staff called my wife and suggested the secure wing. • ~1 month later: He escaped through a window, a “secure” gate, and over a fence. • 4 hours before the facility called us or police. • Police issued a missing persons report; CBI was notified. • Days later, Director Luis found him at a downtown homeless shelter. • No ambulance. No transport back. No medication. Luis said he “couldn’t force” him to return—despite his medical needs. We told Luis: “That’s why he’s in your care.” He did nothing. My sons lent their uncle a Nintendo Switch + games to cheer him up. After the escape: • Gone from his room. • Police body cam footage (confirmed by the officer) never showed it with him or at the scene. • Luis claimed police “saw it in his backpack.” The officer said no. Luis refused to investigate or replace it. Bottom line: • “Secure” wing = not secure. • Life-threatening delays. • Missing property, zero accountability. • Director Luis shrugged off everything. Do NOT send anyone you love here. They failed at safety, care, and basic honesty.

New Management Just moved in and It is looking Promising. Before than I would Say it is Average. There are some amazing staff here that truly care for the residents, and some who just want a paycheck. Stars of the place are Jean, Abby, Letty, Noella, Erica, Jamie, Kevin, and a handful of others. Kitchen Staff Maria and Carmen are great. Everyone in the Therapy Department Rock. Issues, Need a supply director/coordinator, Need a more diverse Menu selection, and Need to listen more to the residents needs as for who they work with and trust. As Residents we all have our special needs routines and quirks and those who we feel comfortable, safe, and trust should be scheduled more with us rather than outside agency personnel. Place is clean and house keeping does there best. Laundry, they need a 3rd shift laundry person to keep up with the linens.
