I would NOT recommend the rehab facility. The CNA staff and rehab staff have been great. Although the CNAs were sweet and as helpful as possible they do not have enough staff to assist patients as needed. I would come in my mother would be wet, bed would be soiled and sometimes she wouldn't have on an adult brief. We would have to ask them to change her. We would have to come in brush her teeth and clean her up often. When you walk in the facility it reeks of urine. The food is bland and thrown together. The case manager Margarita is not helpful, and is very short with families asking clarifying questions.She doesn't give updates and has an attitude via telephone, she's speaks as if she doesn't want to be bothered. The nurses seems to never know what is going on with the patients in the facility. You ask them a question that put it off on the next person. My mother ending up with a hole above her buttocks and the wind care nurse didn't treat it properly. During discharge we had to find transportation for my mother to be transported on our own. They will let your loved one sit there in pain, especially if your loved one is non verbal. She had to a soft diet and no one who assist her to eat they would just give her food and leave her which was definitely a chocking hazard. Beware. If you choose this facility you must be as present as often as possible and be a big advocate for your family member.
All I can say is Wow! This building tours so well and the Director of Nursing was so compassionate From the Admissions team to Discharge Planning everything was seamless. I will always refer to this facility.
I spent spent several months in a rehab facility, Signature Healthcare of Primacy. That was an experience I will never forget. The facility was clean, many of the staff was exceptional, food fair and moments of pure goodness (after I complained)and the rehab was outstanding. I want to thank the angels who was exceptional in assisting me and coming to my rescue...My therapist Kwana Gilbert & Aadelighn Ivy. My outstanding nurse Jessica Howard,dietary Shawanda and Demorio. All shifts CNAs were remarkable. Evening CNAs and 3rd shift CNAs especially Chinthia who would just pop up at my worst moments. There are too many exceptional CNAs to call out. All the staff who worked with the residents were truly wonderful. The upper managent (CEO) was not genuine at all. I had the impression it was fake and false. They would make their appearance as if it was a press conference. Serve a few coffees, make a few beds and disappear. TALK AT YOU NOT TO YOU. Somewhat making you feel as if you are leas than. When talking to them there is no compassion and always in ther favor. They were always quick to respond when called on. Did t see my Social Worker after an all desipline meeting at admission week. I saw her right before discharge. Other deciplines made efforts to improve and followed up. Dietary responded quickly to my requests and followed up. Shawanda and Demario from Dietary made efforts to accomdate areas that needed improvements. The facility had a lot of polices that are questionable. Such as no matter what during meal times no other services are done. All focus is on meals and meals only. Getting them served and picked up. Call lights are not always answered if so, they are not carried out. I waited an hour and a half one time. Just to be put off. Which is the usual response. So if your call light is answered does not mean you are getting the response you need. But amazingly God would always send me an ontime angel to assist. SO if you are not patient and can not advocate for your self this is not the place to go. Overall it was aplace I appreciate in some areas and dislike in others. Don't expect a good customer based facility, not from management. Not the best customer friendly place. Exceptions, the rehab staff are very customer friendly and oriented...the best! Yes there were many good staff members there. Thank you all!
i thought since we was all the same color they would take care of my auntie but they didn’t tend to care for her, they kept my auntie dosed up the whole time, they wasn’t brushing her teeth, changing her or anything and i would say some comments i heard in the hallway but this about my auntie and my auntie looked like she was on her death bed the way they kept her dosed up, then had the nerve to lie and say she was asking my auntie couldn’t talk and my auntie was talking perfectly fine before she came to this whatever you want to call it just don’t send your people here if you want your people to come out alive. Zero stars!!!
