Citadel Hospice is a Medicare-certified hospice provider serving Montclair, California and surrounding areas. A Medicare-certified hospice provides comfort care and support services to patients with terminal illnesses and their families. Hospice care focuses on quality of life rather than curative treatment.
Hospice care is available to patients whose physician certifies they have a terminal illness with a life expectancy of six months or less if the illness runs its normal course. Patients may receive hospice care at home, in a hospice facility, nursing home, or hospital.
Medicare-certified hospices must provide all four levels of hospice care: routine home care, continuous home care for periods of crisis, general inpatient care for symptom management, and inpatient respite care to give caregivers a break. Care is provided by an interdisciplinary team including physicians, nurses, social workers, chaplains, and trained volunteers.

Boy what a godsend the folks at Calstro are. Especially Alex. He came to the hospital to talk with my dad and sister and explained everything about hospice to us. He was very patient, understanding and so empathetic. My dad has moved a couple of times since the initial intake and they are right there, the day he moves. I’m truly impressed with how they keep up with everyone. Although there are social workers, etc, I have always turned to Alex when I have a question and he has been so patient with me. Answering questions, directing me to the right person. He is just an awesome guy and I don’t think my dad or me and my sister would have navigated this journey as well as we have without him and all the nurses and home health people. Thank you seems so small for all the work and type of work they do
It’s really difficult to select a hospice provider that will deliver quality service and communication and you will definitely find it with Calstro Hospice. May and her team are great partners in the community and make the transition smooth for all those seeking hospice and respite care. Highly recommend!
Nursing staff was wonderful to my husband but when it came to the end of the fiscal year he was dropped suddenly with 5 days notice because they needed to meet their budget. Unacceptable... never trust them!
Calstro Hospice went above and beyond in taking care of my grandfather. Their liaison May, is charismatic, charming and compassionate. She truly went above and beyond and helped us every step of the way during an extremely difficult time. Their nurses were quite knowledgeable and anticipated my grandfather’s needs before they were even know. They were always available when we needed them and certainly exceeded our expectations. I highly recommend them.
The care that the assigned staff of Calstro Hospice gave to my dear sister was excellent. They kept in touch with me often and explained her condition and what steps were taken to give her the best possible treatment, which relieved me greatly when I couldn’t be at the nursing facility. All were compassionate with her and me. Thank you to Calstro Hospice for the support and service given to my sister in her end of life needs.
