The primary responsibilities of the home health aide include the performance of basic personal care procedures under the supervision and direction of the RN according to he written patient care plan. Responsibilities will include the provision of personal care; assistance with rehabilitative care and activities of daily living; performance of household services deemed essential to the patient's health care in the home; observing and reporting changes in the patient's condition; and completion of the required clinical notes and records.
