Licensed Vocational Nurse (LVN) Job Description for Vista Adult Day Health
1. Qualifications:
a. LVN license by the State of California.
b. Write and speak fluent English and Spanish.
c. Demonstrate and understanding of health issues of the elderly.
2. Experience:
a. At least two (2) years experience in a community health setting.
3. Duties and Responsibilities:
a. Monitor status and progress of program participants and recommends appropriate levels of care for those participants with needs.
b. Provides skilled supportive and restorative care.
c. Responsible for the administration of medications and training in self-medication.
d. Responsible for the health areas of orientation and in-service training for staff, volunteers and participants.
e. Provide health education and counseling to participants and their families, if indicated.
f. Defines the responsibility of licensed nursing staff and program aides, and provides supervision.
g. Provides evaluation to determine type of emergency medical services required.
h. Acts as a member of the multidisciplinary team and participates in all case conferences, if needed.
i. Participates in quarterly Utilization Review Conferences.
j. Daily charting for routine services may be done with a flow chart, which can be checked off and initialed.
k. Records, signs, and dates progress notes where there is any significant health change, and ongoing absence or refusal of services and quarterly progress notes.
l. Work with participant’s physicians and therapists to ensure appropriate care of program participants.
m. Or any other duty assigned by the supervisor and/ or Program Director
