Certified Nursing Assistant (CNA) Job Description
We are seeking compassionate, reliable Community Participation Support (CPS) Professionals to support adults with intellectual and developmental disabilities (IDD) in meaningful community-based activities.
This role focuses on engagement, routine, and quality of life—helping individuals safely participate in their communities while working toward goals outlined in their Individual Support Plans (ISPs).
This is not a day program and not babysitting. Our staff provide intentional, individualized support that promotes dignity, choice, and inclusion.
What You’ll Do
Support individuals during community-based activities such as:
Walks, parks, libraries, stores, and community outings
Sensory-friendly and wellness activities
Volunteering or social engagement (as appropriate)
Follow each individual’s ISP goals and daily routines
Encourage choice-making, engagement, and emotional regulation
Provide hands-on support as needed (varies by individual)
Transport individuals safely to and from activities (if required)
Complete brief daily documentation of activities and progress
Communicate professionally with supervisors and families
