Elements of Elder Care is looking for multiple East Valley Hospice Aides with a Signing Bonus of $500 to join our team!
The Hospice Aide will:
- Provide patient care in the East Valley only - Mesa, Chandler, Apache Junction, Queen Creek, San Tan Valley areas with mileage reimbursement.
- Provide exceptional care according to the patient's personalized plan of care. Meet patient frequencies and tasks lists.
- Document patient visits in the Consolo charting system.
- Drive to and provide care wherever the patient calls home. This includes group homes, facilities and private residences.
Requirements:
- High school diploma or GED.
- Certified or licensed by the AZ Board of Nursing.
- CPR certified.
- Valid Driver license
- Current auto insurance.
