Home Care Aide (HCA) Job Description
SUMMARY: Implements training and support services to adults with intellectual disabilities and special needs living independently. Empower consumers by providing support they need to live safely in their own home, neighborhood, community and job site. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following 1. Implements training and support services in all areas of support plan as outlined in the Individual Service Plan (ISP). Training, assessing and support includes, but is not limited to, meal planning, shopping, cooking, laundry, personal care (bathing, grooming, dressing, etc.), peri-care (toileting, cleansing, etc.) , budgeting, independent living skills training, self-help skills, recreation and leisure planning, banking, decision making, self-advocacy and problem solving and mobility as indicated by the consumer’s need. In-home housekeeping duties are not included. 2. Participate in the development of the ISP by providing input for service planning and implementation of services as they are developed and modified. 3. Participates in circle of support meetings; staff meetings and in-service training as required. 4. Assists consumer in accessing community resources, by utilizing your personal vehicle and/or training in the use of public transportation (MTA, ACCESS Paratransit Services, Taxi, Uber, etc.), for further development of independent living skills in the area of community access. 5. Reports any change in condition, physical and/or emotional health, immediately to the supervisory staff. 6. Supports consumer with their medications and over the counter medication, as needed, by ensuring that a 7 day supply of medications is available at all times. 7. Maintain professional boundaries with consumers at all times. 8. Demonstrates a respectful attitude toward the consumer; allow them to have as much independence as their potential allows. 9. Overnight positions require bed-checks every two hours to ensure the health and well-being of the consumer. 10. Supports the consumer with behavioral development by implementing intervention strategies to redirect maladaptive behaviors as necessary. 11. Assists consumer with medical/dental appointments under the direction of the supervisor 12. Assists consumer with resolving In-Home Support Services, Social Security Benefit, Section 8, Low Income Voucher, etc. concerns under the direction of the supervisor. 13. Assists consumer with applying for low income, reduced rent and/or Section 8 applications; participates in scheduling, attending appointments and interviews under the direction of the supervisor. OTHER DUTIES AND RESPONSIBILITIES include the following: 1. Completes and timely submits all necessary paperwork, including daily progress notes for all consumers supported. 2. Completes and submits legible timekeeping and mileage documentation. 3. Participates and completes all staff training components; including maintaining current First aid and CPR certification. 4. Maintains accessibility to consumer served and New Horizons staffs by maintaining an active cell phone. 5. Believes in and supports the agency’s mission. Works effectively with co-workers, consumers and others by sharing ideas in a constructive and positive manner. Listens to and objectively considers ideas and suggestions from others. SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities.
