Here are the requirements: 1. CNA License or HHA Certificate 2. Background Screen level 2 3. CPR Card - Red Cross or American Heart Association 4. OSHA Certificate 5. HIV/AIDS Certificate 6. Alzheimer's Certificate 7. Domestic Violence Certificate 8. Assistance with Self - Administration of Medication Certificate 9. HIPPA Certificate 10. Professional Liability Insurance (only required if the client requests it) 11. Drivers License or Florida ID 12. Auto Insurance 13. Social Security card 14. Proof of Residency - Work Permit/ Alien Card OR Citizenship, Passport 15. Current Physical with Chest X-Ray (done within the past 5 months) 16. (2) Verification of Employment OR (1) Verification of employment and (1) Reference letter
