Provides personal care and related services in the home, under the direction & supervision of the Staff Nurse or the Director of Patient Care Services.
Follows the Care Plan in providing a safe and competent care to the patient.
Assist the patient with Ambulation , bathing as ordered by the Physician.
Performs routine housekeeping task as instructed by the RN.
Prepares visit report promptly and incorporate in the clinical record weekly.
Attends in-services as required by the regulation.
Must be able to drive to patients home.
Must be able to communicate clearly, both verbally and in writing.
