PRN Nurse (LPN / RN)

Job Summary Xclusive Care Services is seeking dedicated and compassionate PRN Licensed Practical Nurses (LPNs) to join our home health team. In this role, you will provide skilled nursing care and support to patients with diverse medical needs in their homes. This is a flexible per diem opportunity, ideal for nurses who are passionate about patient-centered care and prefer schedule flexibility. Responsibilities Provide skilled nursing care in accordance with the patient’s care plan and physician’s orders Administer medications and treatments accurately and safely Perform wound care, injections, catheter care, tracheostomy care, and g-tube feedings (if applicable) Conduct patient assessments and report changes in condition to the physician and agency Support patients with chronic conditions, disabilities, or complex medical needs (including pediatric and adult populations under GAPP and ICWP) Educate patients and families on disease management, medications, and home safety practices Collaborate with physicians, therapists, CNAs/PCAs, and family caregivers to ensure continuity of care Maintain accurate documentation and timely reporting in compliance with state and agency requirements Adhere to infection control, safety standards, and HIPAA regulations Required Documents (Must Be Valid/Current): Driver’s License Social Security Card CPR Certification (Required) Active LPN or RN License in the State of Georgia (Required) TB Test Result (within the past year or within 7 days of hire if not current) Background Check (instructions provided upon hire) Qualifications Licensed Practical Nurse (LPN) or Registered Nurse (RN) with an active Georgia license Prior home health, pediatric, or adult disability care experience preferred Strong clinical assessment and critical thinking skills Excellent communication and documentation abilities Compassionate, reliable, and able to work independently in patients’ homes Benefits: Flexible PRN schedule to match your availability Opportunities for professional growth and development Ongoing training and support Employee recognition programs Job Types: Part-time, Per Diem (PRN) Specialty: Home Health Work Location: In-person (patients’ homes in the Atlanta, GA area) Job Types: Part-time, PRN, Per diem Benefits: Flexible schedule Work Location: In person
