Community Home Health Vocational Nurse (LVN)

Job Summary: We are looking for a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our home health care team as a PER DIEM nurse. The ideal candidate should have a solid nursing background and experience in providing care across various medical settings, particularly in home health, internal medicine, or inpatient care. As a home health LVN, you will play a vital role in delivering personalized, high-quality care to patients in their own homes, assisting with medical procedures, and supporting their overall health and well-being. Responsibilities: -Provide hands-on patient care, including administering medications and monitoring vital signs. -Assist patients with daily living activities and performing wound care. -Provide comprehensive patient education on managing health conditions, and lifestyle modifications in a home setting. -Accurately document patient information, care plans, and condition. -Coordinate patient care and facilitate effective communication within the home health team and medical staff. -Coordinating personalized patient care plans with home health team members. Requirements: -Active and valid Licensed Vocational Nurse (LVN) license. -Reliable transportation & traveling with mileage reimbursement -Spanish-speaking skills preferred -Previous experience in internal medicine, or hospital medicine is preferred. -Excellent communication skills and the ability to collaborate effectively with a multidisciplinary team. -A compassionate and empathetic approach to patient care, focusing on supporting patients’ individual needs. -The ability to adapt and thrive in dynamic, fast-paced home health care environments. -Strong attention to detail and ability to maintain accurate records. -Familiarity with case management practices is an advantage. -Join our team to make a difference in the lives of our patients while advancing your nursing career!
