Home Health Per Visit Field Nurse in Garden Grove, CA

Our team is looking for a HOME HEALTH FIELD LVN. We currently have active patients in the Garden Grove, CA . Our patient load consists of medi-cal state insurance patients needing wound care, medication management, dmii teaching and ostomy/foley care. CNE Home Health does a daily schedule that is emailed that day before by 5pm. On days that you are available to work in the above city the scheduler will be able to assign you about six to eight patients. The field lvn participates in patient and family teaching, communicates and works in conjunction with other disciplines in promoting and maintaining independence and optimal health to homebound patient. The field lvn is an integral part of the interdisciplinary team and will be giving report to the office nurse to provide the best plan of care for the patient. New graduates/New to home health welcome. Paid hourly rate for orientation.
