Constant Care Home Health Inc. is seeking compassionate, reliable, and experienced Registered Nurses (RNs) to join our growing home health team. We provide skilled nursing services to patients in the comfort of their homes throughout Los Angeles County, Metro LA, Orange County, Oxnard, Bakersfield, Palmdale, San Fernando Valley, and surrounding communities. Responsibilities: Conduct skilled nursing assessments and patient evaluations. Perform wound care, medication management, and other skilled nursing procedures. Educate patients and families regarding treatment plans and disease management. Communicate patient progress and changes in condition to physicians and the clinical team.
