JOB DESCRIPTION - Performs and completes comprehensive assessments of patient status including OASIS DATA SET. Initiation of service reassessment and discharge. - Responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured or infirm and the maintenance of health and prevention of illness of others including initiating appropriate preventive rehabilitative nursing procedures. - Act as the coordinator of the health care team in order to maintain the proper linkages within a continuum of care. - Practices nursing with respect for individual, cultural, and spiritual differences. - Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in home care situations. - Performs other duties as assigned. - Discharge planning. REQUIREMENTS - Current license as a Registered Nurse in the state of Illinois. - Must have a valid driver's license. - Strong communication and interpersonal skills. - Minimum of one (1) year experience in an acute care setting or equivalent experience.
