HOME CARE VISITING NURSE

Brooklyn based Home Care Agency looking to hire Full Time and FFS Home Care Visiting Nurse. Provides quality care to patients in home care setting. Conducts a thorough and precise assessment of patient condition, functional abilities, home bound status, and environment and social factors. Makes appropriate referrals for additional services such as PT, OT, MSW, HHA. Provides care in accordance with physician orders, the patient's condition, accepted standards of practice and Agency policies. Identifies changes in patient conditions, consults with patient care team and implements appropriate interventions. Develops and implements nursing care plans and discharge plans with involvement of patient and patient care team. Must complete paperwork properly and timely including OASIS and visits .RN must have Home care experience, Bachelors Degree Preferred, Computer Experience, and Malpractice Insurance, willing to travel.
