1. Provide admission, case management and followup skilled nursing visits for hospice patients.
2. Addressing and supporting clinical concerns as part of office team.
3. Supervisory visits to observe clinical proficiences a field staff while w/ patient
4. Educate patients, families & caregivers
5. Provide skilled nursing as determined by the interdisciplinary plan of care.
Compensation
$52 - $57/ hour
