Join our compassionate care team across more than 40 skilled nursing communities throughout Iowa. We are seeking an experienced and organized Admissions Nurse (RN) who will serve as a key clinical leader in the admission process while also providing charge nurse support and clinical oversight when admissions are not occurring. This role is ideal for an RN who enjoys building relationships, coordinating care transitions, and ensuring a seamless admission experience for residents and families while maintaining excellence in clinical operations.
What You'll Do & Key Responsibilities:
- Lead and coordinate all resident admissions, including clinical assessments, documentation, and intake procedures.
- Serve as the primary clinical contact for incoming admissions, families, hospitals, and referral partners.
- Review admission paperwork and ensure accuracy and compliance with state and federal regulations.
- Complete admission assessments and collaborate with the interdisciplinary team to develop resident care plans.
- Perform wound assessments, basic wound care, and clinical evaluations as needed.
- Complete lab draws.
- Assist as a charge nurse and with med pass as needed.
- Monitor resident conditions and respond promptly to changes in status.
- Administer medications and treatments accurately and on time.
- Support daily clinical operations on the unit, ensuring quality care and resident safety.
- Collaborate with CNAs, LPNs, providers, therapists, and other interdisciplinary team members to ensure high-quality resident care.
- Communicate effectively with residents and families regarding care needs and transitions.
- Document care accurately to meet clinical, regulatory, and facility standards.
- Follow infection control, safety, and compliance protocols at all times.
- During periods of low admission activity, assist with nursing unit operations, charge nurse responsibilities, resident care coordination, audits, and other duties as assigned to support the facility's clinical needs.
- Review referrals and coordinate required equipment and supplies for resident care.
Qualifications:
- Valid RN license in good standing.
- License must be valid within the state of Iowa or have the ability to activate.
- Current, valid CPR Certification.
- Previous experience in long-term care, skilled nursing, admissions, case management, or care coordination preferred.
- Knowledge of federal and state long-term care regulations and laws.
- Strong organizational, communication, and time management skills.
- Ability to work independently while managing multiple priorities.
- A strong commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.
