-Conduct initial assessment /reassessment and quarterly assessment for participants and develop the care plan accordingly.
-Conduct wellness and risk assessments including Covid-19 screenings.
- Observe and monitor participants' health conditions and changes.
- Instruct/ train participants and families on health issues, precautions and medication management.
- Participate in multidisciplinary team (MDT) meetings.
-Communicate with participant’s family/caregiver and primary care provider on changes in participants’ health status.
-Respond to all medical emergencies and take the lead in management of the situation as indicated.
-Perform other related duties as assigned.
Bilingual in Spanish is preferred.
