Hospice RN Case Manager, $10K Sign In Bonus, Full Time and Part Time Positions

An Elite Health Care Systems Registered Nurse (RN) Case Manager is a customer service professional who provides end-of-life care for patients with life-limiting illnesses. RN Case Manager is an experienced registered nurse who initiates and coordinates the hospice care plan, supervises CHHA’S and LVN’S and provides skilled nursing hospice and palliative care services to our patients. The RN Case Manager will work cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary hospice team. Must be able to utilize computers and be comfortable with EMR.
