Registered Nurse (RN) Job Description
Urgently hiring Nurses for home health services. Immediate openings available for Full and part time Registered Nurses to perform health assessments for clients in their home to accurately assess a patient's health care status and physical needs, educate on disease process and Medication set -ups and reconciliations each visit. Visit occurs Monday through Friday between 8am and 5pm according to patient’s need. Our agency is willing to train newly graduate registered nurses who are not afraid to utilize their skills/competency. Divine Home Care offers flexible scheduling of 1-2 days/week. ESSENTIAL DUTIES AND RESPONSIBILITIES (Includes the following; other duties may be assigned)
o Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
o Perform the initial home care patient visit and re-evaluate the patient’s needs and progress on a regular basis. Initiate the plan of care under doctor’s orders.
o Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient. Observe, assess, and document symptoms.
o Monitor reactions and patient progress. Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
o Coordinate patient services. Notify the physician and other personnel (DON, PT, Case Manager) of change in the patient’s condition. *
o Perform skills like wound care, med sets and others outline in the agency’s approved policy and procedure manual.
o Nurses will communicate with providers to to ensure coordination of care. RN to provide onsite supervision of LPN/HHA*
o Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually.
o Adhere to Medicaid and Medicare regulations.
