The Skilled RN: Travels to patients’ homes to complete an initial assessment, create the care plan to determine home care needs, and provide direct patient care. Includes the patient and the family in the development of the care plan and counsels them on how to meet the goals put in place. Regularly re-evaluates the patient’s nursing needs and supervises LPNs or HHAs working with you to care for the patient. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Maintains appropriate clinical records and notes, and reports to the physician any changes in the patient's condition. Submits these records in accordance with the Organization's policy. Collaborates closely with a multidisciplinary team, physicians, and other wellness professionals to ensure a seamless and supportive care experience for every client. Must have experience with Home Health, OASIS documentation, and CMS regulations. Proactive, resourceful, and reliable team player with good verbal and written communication skills. Must be a licensed driver with an automobile that is insured and is in good working order. Possesses and maintains current CPR certification. Can provide a copy of a negative TB test.
