SUMMARY Solaris Home Healthcare, Inc is The ACHC accredited home health care agency in Northbrook providing exceptional patient care since 2010. We are patient centered home health care agency looking for a Part Time/Per Diem RNs in Cook/Lake/McHenry Counties. As a visiting nurse, you will create your own work schedule, provide supervision to LPN's and CNA's, and work closely with physicians, patients, caregivers to develop care plans to facilitate the best possible patient outcomes. ESSENTIAL DUTIES AND RESPONSIBILITIES Candidates must have strong spoken and written communication skills and be a dedicated team player. Develop and implement a plan of care that meets each patient's specific needs according to supervising MD orders and with agency policies and procedures. Continually communicate patient care to office staff, involved clinicians, and supervising MDs in making decisions regarding patient's care, coordinates family involvement in the assessment/treatment and continuing care for an assigned caseload of patients Be responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching for patients; Maintain a clinical record for each patient receiving care; Prepare written instructions for the aide, and supervise the aide in the home; Direct the activities of the licensed practical nurse; Administer medications and treatments as prescribed by the patient's physician or podiatrist; Participates in in-service programs, which continually educates clinical staff on changes in policies and procedures. Act as the coordinator of the health care team in order to maintain the proper linkages within a continuum of care. QUALIFICATIONS: • Active Registered Nurse license in the State of Illinois • Experience in home health care setting preferred • Basic Life Support (BLS) certification required • Must have a vehicle and proof of auto insurance • Valid driver's license and clean record • Electronic Medical Records experience a plus • Computer skills
