As the RN Case Manager, you will plan, organize and direct hospice care for your patients. This professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. You will be the central point for the patients care plan and will help lead and mentor our Hospice Aides. Our Case Managers will provide care through our service area.
