Registered Nurse (RN) Case Manager
Coordinate total patients care by conducting comprehensive health and psycho social evaluation, monitoring the patient's condition, promoting sound preventive practices, coordinating services and teaching and training activities. RN to perform IV therapy, medication management and wound care. Coordinates home health and hospice care by working with other team members as well as with the patients / family members to ensure needs are met.
