You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.
Pay per Visit
- Sup/Post Hospital: $60
- Reassessment: $75
- Initial Admission: $90 weekday/ $125 weekend
Schedule:
Services will be provided two weekdays per week during daytime hours, with one of those days conducted on-site. Visits will be scheduled to coincide with the patient's aide's required presence.
Mileage Reimbursement: Provided at the current federal rate
You will...
- Performs on-site supervisory and teaching visits to evaluate caregivers
- Provides clinical orientation to caregivers on assigned home care cases.
- Participates in the training of new nurses and in-services for field staff.
- Performs initial nursing assessments
- Counsels patient/caregiver in meeting nursing and related needs.
- Assigns and coordinates services provided.
- Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.
- Prepares clinical progress notes ensuring any changes in the patient’s condition is reported
- Monitors clinical records for complete and accurate documentation.

- CPR Certified
- 1 year of Home Care experience required
- 1 year of supervisory or management experience required
- Valid NYS Driver’s License, insurance, registration, and reliable car
