Our services include short and long-term rehabilitation, skilled nursing, adult family homes, mental health, memory care, and home health. Responsibilities
Coordinate management of client’s outpatient mental health benefits
under PHP for persons assigned to primary caseload including monitoring of individual treatment plans and achievement of treatment goals.
Assist in establishing and implementing procedures for medication storage, reordering, administration and client compliance.
Administer medications as directed. Observe for effectiveness and side effects.
Perform routine assessment of physical complaints; monitor ongoing medical problems and make client referrals for medical emergencies and routine follow-ups.
Maintain and enhance customer service to ensure that both external and internal customers of the facility have a positive experience.
Comply with all state WAC requirements and facility policies and procedures.
Ensure assigned staff complies with all state WAC requirements and facility policies and procedures.
Perform facility charting requirements, correspondence, SALs and other paperwork in a complete and timely manner.
Perform all other nursing duties as assigned.
