Position Overview:
As a Home Health RN, you’ll be responsible for planning, coordinating, and documenting care for an assigned caseload of patients. You will work closely with physicians and Case Managers to develop and implement personalized care plans that meet each patient’s specific needs while maintaining compliance with federal/state regulations and agency policies. You'll also actively participate in performance improvement initiatives.
Qualifications:
Active RN license in the State of Virginia (required)
Minimum 1 year of home health experience (preferred)
Valid driver's license with a clean driving record (required)
Strong documentation and communication skills
Ability to pass a background check
Bilingual applicants preferred, but not required
