Job description Responsibilities: Provide medical care for patients suffering from chronic illnesses. Assess patients' physical and emotional condition and develop an appropriate treatment plan. Treat patients and document health problems. Keep patient records. Communicate with other nurses, doctors, and families about patient conditions.
