RN Home Health Case Manager

Our home health agency is looking for a full time RN Case Manager. We are an established, mid size (80-100 patients) home health agency and are looking for a RN with at least 2 years of RN clinical experience. Home health experience is helpful but not required. This is a Mon-Fri (9-5:30pm) Office Position with a rotating On-Call requirement. Responsibilities include but are not limited to: Reviewing OASIS and POC, case managing patient care, coordinating with field clinicians and following up on MD orders as appropriate.
