Caregiver Job Description
Caregivers are responsible for following the client’s Care Plan, which may include assisting with personal care as follows: bathing, dressing, grooming, feeding, exercising, toileting, positioning, assisting with self-administered medications, routine hair and skin care, and transfer and/or ambulation.

Care Managers/Staffing Coordinators/After-Hours Personnel
Minimum Eligibility Requirements:
Must be 18 years of age and demonstrate competency, when competency cannot be determined through education and experience, to perform the tasks assigned by the Care Manager;
Verifiable professional and personal references;
Infection control training preferred;
Proof of current CNA certification, if applicable;
Valid Driver’s License or US identification and proof of insurance;
Have reliable transportation;
Must be physically able to perform the duties of this position;
Be able to follow direction and accurately report to the Care Manager;
Have excellent communication skills;
Read, write, and speak English;
Are mature, caring, punctual, and reliable;
Possess basic computer knowledge and skills;
Pass a DPS criminal background check; and
Pass DADS NAR/EMR Verification of Employability check.
Essential Functions:
Understanding a client’s care plan and adequately implementing it;
Assist clients according to their care plan, which may include: companionship, meal preparation and service, light housekeeping, transportation, shopping, assistance with self-administration of medication, bathing, body care, dressing, feeding, ambulation, positioning, transferring, and toileting;
Notify the Care Manager of client concerns, changes in care or client condition and client emergencies;
Notify the Care Manager and/or Staffing Coordinator of any safety concerns;
Follow company Infection Control practices; notifying the office if in need of appropriate Personal Protective Equipment (PPE) or supplies;
Follow the ClearCare Telephony Instructions to clock in and clock out for all scheduled shifts and accurately update your task status for each client prior to clocking out and leaving the client’s home. For compensable worked hours that are not allocated to a specific client (such as training time), you must use and submit the Log and Certification for Non-Visit Time Worked form for each period pay period.
Note: It is never acceptable, under any circumstances, to clock in or clock out other Agency employees on their behalf. If any Agency employee should ask you to clock in or clock out for them, you are required to decline and report the incident to your supervisor immediately.
Conform to the company dress code, as specified in the Employee Handbook;
Report to work on-time;
Complete ongoing training as required by client care needs and/or changes in company policies and procedures;
Adhering to all company policies and procedures as detailed in the Employee Handbook; and
Attend staff meeting and in-services, as needed/required.
