LPN/RN - Full-time / Part-Time

We are looking to hire full-time and part-time LPN/RNs! Join an empowered, self-managed work team located in Woodland Park Long-Term Care, residents living in real homes. As a nurse and a member of the Clinical Support Team (CST), this position monitors the clinical status of residents and delivers appropriate care. Assumes the responsibilities of care role model, gerontological expert, care team partner, mentor, and teacher to the Shahbazim (CNAs who serve as universal workers in the homes). The short term rehab aspect of patient care is in a post-acute care setting. As a LPN or RN working in Transitional Care, you’ll have the opportunity to be an essential part of helping patients reach goals to help them transition home and/or to a less dependent level of care. As part of the nursing team, you will work closely with other healthcare disciplines to assist each patient as they aim to improve their health and wellness outcomes. As a condition for employment, VMRC requires all employees to be fully vaccinated.
