Licensed Practical Nurse (LPN) Job Description
The licensed graduate nurse provides nursing care and teaching to patients and families. He/She also functions as an assistant to the physician and registered professional nurse, and is supervised by the RN.
Qualifications:
- Graduate of an approved program of practical nursing
- Licensed as a licensed graduate practical nurse in the state where practicing
- One (1) year of experience as a nurse
Responsibilities:
- Participate in the planning and coordination of patient care in conjunction with the
RN and the Physician’s plan of Treatment.
- Follows the nurse care plan and contributes to updating the care plan as needed
- Observes the patient, is able to identify patient problems, evaluates the care of the
patient, and applies their knowledge of skills to the patients, and follows up by
notifying LifeLine Home Health, physician and family promptly of any changes in the
client’s status.
- Performs only those tasks/treatments which are within the scope of practice and/or
for which they are qualified by training and show documentation of competency
signed by an RN, physician, or specific training program director.
- Prepares clinical/progress notes on the day of visit and incorporates it in the clinical
record and sends in required paperwork within the specified time which is due in the
office weekly no later than Tuesday by 5:00 pm.
- Accurately reports and hands off client information to other staff and/or family
member(s). Records the client’s condition using all the systems, current care and
anticipated care, vital signs, pain level, allergies, and symptoms which may be
indicative of change, if client refuses it must be documented as refused.
- Assist the patient with activities of daily living while teaching appropriate self-care
techniques.
- Provides and maintains a safe environment for the patient and self, reports any
safety issues to supervisor as they arise.
- Demonstrates good interpersonal communication skills in working with peers and
other disciplines, and reports any problems promptly to the supervisor.
- Respects the privacy and dignity of patients and their families.
- Attends on-going in-service education or completes in-service continuing education
as required by the state licensing board.
- Provides proof and maintains current documentation of the following: PPD/CXR,
CPR, driver’s license and proof of auto insurance coverage for those employees
transporting clients and person liability insurance for contractors
- Reports to assignments on time, and if unable to report to an assignment, employee
will notify LifeLine Home Health promptly.
