JOB DESCRIPTION: In-home Caregiver
SUPERVISOR: DON/Nurse Supervisor
I. Qualifications:
1) Be eighteen (18) years of age.
2) Demonstrate ability to read, write, and follow a written client plan of care document tasks performed.
3) Demonstrate dependability, tact, patience, and the ability to follow instructions.
4) Demonstrate good interpersonal communication skills.
5) Clear Background/Criminal History
6) TB Test
7) Completion of core skills competency By Accessible RN
8) Blood Borne Pathogen/Infection Control Training
9) No findings on NC Health Care Personnel Registry
II. Primary Purpose of Position:
The In-home Caregiver will be a team member of Accessible Home Care providing limited hands-on
assistance. The In-home Caregiver will provide assistance with personal care such as bathing, grooming, meal
preparation and housekeeping services. Accessible RN will verify core competency of In-home caregiver
duties.
III. Other Responsibilities and Duties:
1) Receives client assignment from Accessible Home Care RN with all necessary information pertaining to
service requirements.
2) Receives instructions for duties from the DON/Nurse Supervisor of Accessible Home Care regarding care
plan.
3) Performs all assignments and keeps accurate records.
4) Performs duties to include limited assistance with personal care (eating, toileting, bathing, dressing,
mobility, personal hygiene, medication reminder, that require 50% assistance or less for any hand-on
activity. May also include housekeeping, home management tasks, cooking, running errands for groceries
or medication.
5) Reassures and comforts clients as needed.
6) Encourages proper nutrition assisting client with meals by guiding limbs when assisting client to eat. The
In-home Caregiver may prepare meals for the client when so requested.
7) Keep supervisor informed of any problems regarding client, family, or home environment.
8) In-home Caregiver will contact supervisor when not able to perform duties as assigned due to client refusal
or other circumstances.
9) Maintain professional attitude at all times when on duty.
10) Maintain client confidentiality and follow all policies and procedures of Accessible Home Care.
11) Maintain accurate records of time worked and travel involved.
12) Attend in-service education programs provided by Accessible Home Care unless otherwise determined by
management.
IV. Performance Requirements:
1) In-home care experience of at least one year preferred.
2) Must have own transportation and be willing to travel to client's homes.
3) Must be willing to provide care to all clients regardless of race, religion, ethnic background or diagnosis.
V. Work Environment and Conditions:
1) Accessible Home Care of Greater Charlotte office is located at 130 Ostwalt Amity Road, Suite C Troutman,
NC 28166. Employees will spend approximately 5% of their time at this office.
2) Approximately 95% of the employee's time is spent in the field which includes driving and in the client's
home.
3) Possible hazardous driving during winter months. Employees will abide by Accessible Home Care.
Inclement Weather Policy.
4) Possible hazardous conditions may exist in client homes. The manager will inform the employee of these
conditions when noted during the initial visit. Employee should abide by Accessible Home Care's Employee
Safety Policy.
VI. Physical Effort Requirements:
1) Physical effort may be great in maintaining client safety, changing bed lines, and other housekeeping and
personal care duties.
