RN Case Manager - Field Supervisor - FULL TIME or PER DIEM

Eden HHC is looking for experienced Registered Nurses to join our dedicated team. The candidates must have minimum of 1 year professional home health nursing experience. Our service area includes Los Angeles County, Orange County, Ventura County, San Bernardino County and Riverside County. We use Kinnser/Wellsky for EMR. Position available is for Full Time. Candidates must have: Current active RN license in the state of California Current CPR, Health clearance, TB clearance Minimum of one year of RN work experience, home health preferred Must pass criminal background check Driver’s license without restrictions Proficiency in OASIS charting and timely submission of documents, will train if needed Proof of COVID-19 vaccinations Responsibilities: Complete initial and on-going comprehensive assessments of the patient’s needs Provide service and /or treatments requiring substantial and specialized nursing skills Initiate appropriate preventative and rehabilitative nursing procedures and therapy referrals Educate and support the patient, family and caregiver in meeting nursing and related needs Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
