Registered Nurse (RN) Job Description
We are the fastest growing Home Health Agency in Southern Florida who has the need for a Clinical Case Manager. We are located in Boynton Beach right off I-95 and Woolbright Rd.
We are looking for a team player in a fast-paced fun filled environment.
The Clinical manager provide oversight of all patient care services and personnel.
Oversight should include the following:
Is responsible to makes patient and personnel assignments, coordinates patient care, coordinates referrals, assures that the patient needs continually assessed and assures that the patients' needs are continually assessed and assured the development, implementation and updates individuals plan of care. Also, is a oversea professional staff who is accountable for maintaining the quality of patient care in accordance with the agencies objectives and policies, through planning, coordination, implementing, and evaluating the home health services provided.
Qualifications: A person who is a Registered Nurse. Must be available to the Agency during all operating hours in-house. Must be carry a valid verified licensed in the state.
Skilled, Abilities:
Knowledge of Insurance, Medicare and Medicaid guidelines. Good Communication skills and ability to establish good rapport with other staff members. Working knowledge of home health care and principles, ethics and techniques of professional nursing and requires documentations thereof. Skillful in organization of work and in principles of time management: and knowledge of management process. Ability to contribute to the quality of care being rendered through constructive communications with nursing managers and nursing staff.
Experience: One or more years in home health Agency as a Clinical Manger or Director of Nursing. Spanish speaking is a plus but not required.
