HOME HEALTH/ADVANTAGE PROGRAM (FIELD REGISTERED NURSE & LICENSED PRACTIAL NURSE) POSITION SUMMARY: We are seeking a highly, self-motivated, outstanding RN/ or LPN that has compassion and drive, and strives to provide the best care possible for their patient. DUTIES/RESPONSIBLITIES INCLUDE: Completing assessments, intakes for Start of Care, Discharges, Resumption of Care, Re-certifications, administration medications and treatments, create care plans and adhere to all state and federal regulations. Make supervisory visits to elderly and disabled clients on Advantage and other Medicaid funded programs. RN/ or LPN will be required to complete documents in EHS (electronic health system). Coordinate services with case managers, discharge planner, Position requires someone who is self-directed and has strong organizational and time management skills. Position is paid per visit with mileage. QUALIFICATIONS/MUST HAS: · Must have current RN or LPN license for State of Oklahoma · Must pass full background check · Reliable transportation along with valid driver's license and proof of auto insurance · Current CPR, TB skin test · One year nursing experience (preferred) new nurses do not discourage, if you’re willing to learn; we can train.
