Registered Nurse

Provides professional nursing care as defined in the nurse practice act and under the direction of the Director of nursing. Communicates appropriate status / condition changes to nursing supervisor. Provides skilled nursing visits as directed by the RN case manager. Performs initial and ongoing client assessments (including OASIS data collection) based on agency policy and standards of practice to ensure effective and appropriate home care services. Provide education to patient, families and direct care staff. Collaborates with physicians and other health care professionals.
