Registered Nurse, Home Health (RN)
1. Initiates assessment based on the patients presenting problems/needs in accordance with agency standards.
2. Facilitates patient’s transition to the next level of care.
3. Validates patient/family learning.
4. Electronic document filing for clinical activities.
5. Executes therapeutic and/or supportive interventions.
6. Evaluates the patient/family response to and effectiveness of interventions.
7. Completes OASIS assessment per agency and regulatory policies.
8. Evaluates the patient/family progress toward established outcomes.
9. Follows policies related to MD orders and changes to the Plan of Care.
10. Maintains required competencies.
11. Meets the agency’s productivity standards.
12. Computer competency demonstrated through timeliness and accuracy of documentation.
Requirements
1. California RN license
2. Ability to demonstrate and maintain clinical competency
3. CPR certification
4. Excellent verbal and written skills
5. A positive attitude towards health care team members and diverse patient populations.
