Clinical Supervisor/Delegating Registered Nurse
Vital Sign Home Care is seeking a compassionate and experienced Clinical Supervisor/Delegating Registered Nurse to oversee client care and supervise caregivers in a home care setting.

Responsibilities:
* Complete initial and ongoing nursing assessments.
* Develop, review, and update individualized plans of care.
* Determine which nursing tasks may be safely delegated.
* Train, supervise, and evaluate CNAs, GNAs, HHAs, and Certified Medication Technicians.
* Confirm staff competency before permitting delegated nursing tasks.
* Conduct required supervisory visits and client reassessments.
* Document assessments, care plans, supervisory visits, and clinical updates accurately and promptly in WellSky.
* Review medications, treatments, physician orders, and changes in clients’ conditions.
* Review and monitor Medication Administration Records (MARs).
* Verify that MARs match current physician orders and pharmacy labels.
* Identify and follow up on missed doses, medication refusals, omissions, and medication errors.
* Ensure medication documentation is accurate, complete, and compliant.
* Communicate care-plan updates to caregivers, clients, families, and healthcare providers.
* Promptly report significant changes in clients’ conditions.
* Investigate medication errors, incidents, falls, hospitalizations, and complaints.
* Review clinical records for accuracy and regulatory compliance.
* Assist with infection control, quality assurance, caregiver education, and emergency procedures.
* Remain available for clinical questions and urgent client-care concerns.
* Ensure compliance with HIPAA, OSHA, Maryland Board of Nursing requirements, and applicable Maryland home-care regulations.
Qualifications
* Active and unrestricted Maryland Registered Nurse license required.
* Completion of an approved Delegating Nurse/Case Manager training program required.
* At least two years of home health, home care, assisted living, or skilled nursing experience preferred.
* Delegation and medication-management experience preferred.
* Experience using WellSky or another electronic health record system preferred.
* Strong clinical assessment, leadership, documentation, and communication skills.

Important Role Requirement
The Delegating Registered Nurse must do more than review and sign medication records. The nurse is responsible for assessing clients, supervising caregivers, documenting nursing and supervisory visits, providing appropriate instructions, and immediately stopping any delegated task that is being performed incorrectly or unsafely.
